1. Working group formed to address Rowan Ward issues
(Gateway reference 2740)
Following the CHI report into the issues at Rowan Ward in Manchester where
a group of vulnerable patients in an isolated mental health ward received
treatment that was well below the standards expected, Stephen Ladyman
has sought urgent reassurance that such events will not arise elsewhere.

2. Interhospital transfer audit (Gateway reference
2600)
The Coronary Heart Disease (CHD) Collaborative is undertaking a national
survey of interhospital transfers in cardiac care in all acute trusts.
It will be co-ordinated by collaborative staff to minimise local impact.
The work will last four weeks commencing 1 March 2004.

3. Standardise your crash call number to 2222
(Gateway reference 2667)
The National Patient Safety Agency is issuing a patient safety alert advising
all acute trusts to standardise the crash call telephone number to 2222.
27 different crash call numbers are used in NHS hospitals in England,
causing delay for staff summoning the resuscitation team in an emergency.
With increased staff turnover and more agency/locum staff, this represents
a greater risk to patient safety.
Chief executives are asked to review the risks, costs of conversion and
take appropriate action within available resources. This alert replaces
earlier NHS Estates' guidance (Health Building Note 1997/48, paragraph
2.10).

4. Pre and post appointment checks (Gateway reference
2794)
The National Audit Office's report on suspended clinicians found that
43 per cent of trusts surveyed were not complying fully with the requirements
of HSC 2002/008 Pre and Post Appointment Checks for All Persons Working
in the NHS in England. In particular trusts failed to obtain declarations
from applicants that they are not subject to investigation or proceedings
by a regulatory body. The requirements in HSC 2002/008 are binding on
all NHS bodies under the terms of a Secretary of State direction and failure
to comply with the requirements has serious implications for patient safety.

11. National Programme for IT update (Gateway

The National Programme for IT has launched a monthly programme update.
The update provides a briefing for stakeholders on the programme's progress,
including the NHS Care Records Service, Electronic Booking, Electronic
Transmission of Prescriptions and the provision of an underpinning IT
