Conscious sedation in termination of pregnancy
An expert group has reported to the chief medical officer on the use of conscious sedation in termination of pregnancy.

The report sets out recommendations for abortion service providers who wish to offer, or are already offering, conscious sedation as an alternative to a local or general anaesthetic for early surgical abortion, to those women who meet the criteria.

Testing of tissue donors for HTLV virus
Testing of all new tissue donors for the human T-cell leukaemia virus (HTLV) should start as soon as possible, medical directors have been advised.

Deputy chief medical officer Dr Pat Troop wrote to all medical directors following the recommendation made by the Advisory Committee on the Microbiological Safety of Blood and Tissue for Transplantation.

While the risk has not been formally assessed, it is clear that all organ transplants and most tissue transplants are capable of transmitting HTLV.

Further advice is being sought on:

  • the advisability of testing stored tissue samples
  • the need to test donors of ophthalmic tissues
  • the testing of organ donors.

In the meantime, any stored tissue which has not been tested should, if issued, be clearly labelled as not tested for HTLV.

Safe administration of intrathecal therapy
NHS trusts' compliance with national guidance on the safe administration of intrathecal therapy will be reviewed shortly.

The guidance was issued on 6 November 2001. The Government set a target to reduce the number of patients dying or being paralysed by maladministered spinal injections to zero by the end of 2001.

National alcohol harm reduction strategy
Health and social care staff have been asked for their views on how the Government and others should address the problems of alcohol misuse.

The consultation exercise by the Department of Health and the Cabinet Office will inform work on a national alcohol harm reduction strategy, due to be implemented by 2004.

Patient and public involvement
A consultation exercise has begun on the regulations for establishing the Commission for Patient and Public Involvement in Health. The consultation period will end on 7 November and the commission is due to be established in January.

Patient confidentiality
A consultation on a proposed patient confidentiality management system for the NHS aims to gather views from those who deliver, support, research, regulate and receive services.

The plans propose new ways for staff to work, as well as new tools, processes and technology to support them in sharing patient information.

The consultation will run to 31 January 2003. Organisations and individuals can take part online, through workshops, and through the post.

Local authority scrutiny of health services
Draft regulations on the new arrangements for local authority overview and scrutiny of health services have gone to consultation.

Simpler priorities and planning system
Priorities and a new planning system for the NHS and social services are set out in a key document published last month.

Investment, Expansion and Reform sets out what health and social services will need to achieve in key delivery areas, and introduces a new system of three-year planning and allocations. This will provide a more stable basis for local delivery in national priority areas.

The national targets have been streamlined to include only those targets which will be managed at a national level - with a commitment to avoid the introduction of new targets where possible. The planning system has also been simplified so that each area need only submit a single three-year plan.

Prison health transferred to the Department of Health
Prison health is to become part of the NHS over the next five years.

Primary care trusts will be responsible for commissioning and providing health services to prisoners. Funding will be transferred in April 2003 from the Home Office to the Department of Health, which plans to allocate an extra £46 million a year by 2005/6.

Email and directory for the NHS
A nationally-managed web-based email service to the NHS is being produced, with a complete directory containing details of all NHS staff.

The contract has been awarded to EDS, and the first phase of implementation has begun. The service is being tested in two sites: The Queen Elizabeth Hosptial Trust in Woolwich and a consortium of primary care trusts in the West Midlands.

Safeguarding children
The performance of local arrangements to safeguard children across health, social services, education, police, court and custodial services is the subject of a joint chief inspectors' report, published last month.

The report identified much good practice. However, it found that arrangements to safeguard children were weakened by several factors. For example, some services, including GPs and mental health services, were not sufficiently integrated into local arrangements.

National joint registry for hip and knee implants
Preparation is under way for the national joint registry for hip and knee implants, which will collect information on implants to provide evidence of their long-term effectiveness. A contractor, AEA Technology, has been appointed to develop the registry.

From April 2003, NHS trusts and private healthcare providers will submit data to the registry for all total hip and knee replacement operations.

Stopping violence against NHS staff

Managers' guidance
Guidance for NHS managers on tackling violence has been updated, to reflect developments in the NHS zero tolerance campaign and new information from the Home Office on the Crime and Disorder Act. The updated areas include reporting, police liaison, withholding treatment and prosecutions.

Tackling violence towards GPs and their staff
October 31 was the deadline for primary care trusts to have assessed the risk of violence to GPs and their staff, and to have action plans in place for tackling violence.

The zero tolerance website at www.nhs.uk/zerotolerance now includes examples of best practice in tackling violence in general practice, including local initiatives on provision of secure facilities for treating violent or abusive patients.

Criminal Records Bureau certificates for GPs
The Health and Social Care Act allows regulations to be made requiring a Criminal Records Bureau certificate from GPs applying to join a primary care trust (PCT) medical or supplementary list, and those already on a PCT list.

At present, however, certificates should not be requested. The necessary regulations will not be made until the Criminal Records Bureau is able to perform the checks without the delays currently being experienced.

Tackling health inequalities through local public service agreements
The Department of Health has produced guidance for local authorities who wish to adopt a local public service agreement target on health inequalities.

The guidance includes local authority rankings for life expectancy and examples of health inequalities targets already agreed at local level. It is at www.doh.gov.uk/lpsa/guidance-tackling-health-inequalities.htm.

Emergency care collaborative
The first wave of the new emergency care collaborative was launched on 8 October.

Run by the NHS Modernisation Agency, the collaborative is a national programme for reducing delays and improving the patient and carer experience. It will focus on the whole system of emergency care and will help all accident and emergency departments to meet the NHS Plan target - a maximum of four hours from a patient's arrival to admission, transfer or discharge.

Research into practice summary reports
The NHS Modernisation Agency has produced the first three summary reports of a series highlighting its work on the spread and sustainability of new practice in the NHS. The reports are at www.modern.nhs.uk/redesign.

NHS leadership qualities framework
The qualities to which all NHS leaders should aspire are set out in the NHS leadership qualities framework, launched last month by the NHS Leadership Centre.

A 360-degree assessment tool has been derived from the framework. This can be used to support development for individuals and organisations. The Leadership Centre is working to support roll-out and good practice, initially at early implementation sites.

Report on chronic fatigue syndrome/myalgic encephalomyelitis
An independent working group's report on chronic fatigue syndrome/myalgic encephalomyelitis was published in January 2002. The Department of Health is currently developing a strategy to take the group's recommendations forward.

Pending decisions on this, Action for ME, a leading voluntary organisation active in this field, has produced a useful eight-page summary of the issues, Guidance on the management of CFS/ME.

Keep Warm, Keep Well
Keep Warm, Keep Well is a national campaign to reduce cold-related illnesses and deaths this winter among vulnerable people.

A freephone advice line (0800 085 7000) offers advice on staying warm and keeping well in winter, and information about the financial help available. A free guide, available in several languages, can be ordered from the advice line.

New resource for older people's champions
A new toolkit has been produced in response to calls for increased support for local older people's 'champions'.

The resource is designed to provide champions in local implementation teams with the information they need to support them in promoting the interests of older people, in line with the national service framework for older people.

The NHS quarterly review for April to June 2002 reports aspects of performance such as levels of patient activity, waiting times and numbers waiting, and measures of patient experience standards.

Consultant contract: clarification
Following publication of the framework agreement for the new NHS consultant contract in June 2002, a number of misconceptions have arisen about the framework and how it will work in practice.

A clarification document agreed between the Department of Health, the NHS Confederation and the BMA has been published to help inform discussions at local level between employers and consultants about preparations for the new contract. See www.doh.gov.uk/consultantscontract/index.htm.

National women's mental health strategy
The Department of Health has arranged five consultation and listening events, following last month's publication of the consultation document on the national women's mental health strategy.

Electronic booking event
A one-day event to introduce the NHS to plans for electronic booking will be held in central London on Thursday 21 November, aimed at healthcare leaders.

The new director general of NHS IT, Richard Granger, will be the main speaker. Patients, clinicians and managers involved in electronic booking will contribute.

See & Treat events
See & Treat is a system of streaming services in A&E departments that can have a profound impact on waiting times for patients.

A number of national workshops have been organised around the country that will provide an opportunity to find out more about See & Treat and to hear the first-hand experiences of those who have implemented it.

The events are open to all A&E departments in England and have been specifically designed for A&E consultants, senior nurses and operational managers.

The dates are:

  • November 21, Manchester
  • November 26, Bristol
  • November 28, London

A facilitator has also recently been appointed to help organisations who are facing challenges with the four hour wait target to implement this system of care.

The national day surgery summit invited clinicians and service managers to look at day surgery rates and ways to improve them within their trusts.

Models of emergency care conference
The models of emergency care conference focused on progress being made towards reforming and modernising emergency care, with key speeches from David Lammy MP, Dr Stephen Short and Dr David Dawson.

Dr Short, a Nottingham GP, focused on designing models based on avoiding the need for hospital admission.

Building a world-class workforce in the NHS was the theme of a recent conference addressed by NHS director of human resources Andrew Foster, head of NHS recruitment Liz Eddy, and Judy Hargadon, director of new ways of working at the Modernisation Agency.

Chartered Society of Physiotherapy annual scientific congress
Kay East, the Department of Health's chief health professions officer and director of therapies at East Kent Hospitals NHS Trust, spoke at Chartered Society of Physiotherapy's annual scientific congress. She described how allied health professionals are meeting the challenges of the 'new' NHS.
